JOB SUMMARY
A registered professional who provides coordinated care to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Evaluates occupational roles and occupational performance of patients with physical, psycho-social and/or cognitive impairments. Provides intervention to deficits and elicits responses.
Responsibilities:
- Completes and updates, as needed, a competency skills assessment that accurately identifies their therapy skills. Accepts only therapy assignments that they can perform safely, are within their individual scope of practice and commensurate with their education/experience. Notifies agency leadership immediately if an assigned task must be re staffed due to their lack of education or experience in performing the assigned task. Actively participates in educational opportunities that widen their scope of practice and increases their competency skill set.
- Performs skilled and maintenance therapy treatments on agency patients in accordance with state/federal regulation, the APTA practice act, their state licensure and competency skill set. Performs assessments/reassessments using accepted practices (examples: comparing PLOF vs CLOF, using objective measures). Creates plans of care that are consistent with patient’s illness or injury and particular medical needs, clearly support medical necessity and establishes goals that are tied to function – along with being realistic and meaningful to the patient.
- Identifies changes in patient condition, such as a deterioration in status, and communicates these changes quickly and effectively to the clinical manager, the interdisciplinary team and the physician. Initiates the appropriate therapy response to changes in the patient’s condition and ensures that clear documentation exists within the clinical record regarding the change and therapy response.
- Promptly updates the therapy POC with any new physician orders received. Communicates these changes effectively to the clinical manager and interdisciplinary team.
- Ensures agency’s visit verification process is followed by logging their visits in the patient’s home folder and obtaining a patient/caregiver signature on their daily visit reports (DVR).
Qualifications:
- Current Texas Occupational Therapist License
- Current Driver’s License
- Current CPR Certification
- Home Health experience preferred
