POSITION SUMMARY
Primary function is to ensure that patient visits are scheduled correctly according to the orders on the home health plan of care and to communicate effectively with assigned clinicians regarding this schedule. Provides additional support to the clinical care team, as directed by their supervisor.
Scheduling Responsibilities:
- Utilizes agency established processes to ensure that patient visits are scheduled as ordered on the home health plan of care. Maintains the scheduling book (agency established tool for the management of the schedule) in a manner that allows another staff member to successfully manage the scheduling task in their absence. Ensures that appropriate staff are assigned to all scheduled visits and are made aware of their assignments.
- Understands OASIS scheduling requirements and ensures that OASIS visits are scheduled to be performed within state/federal regulatory requirements.
- Ensures that report is received from RN for all SOC, ROC, RC OASIS and schedules visits according to the plan of care correctly and within a timely manner.
- Verifies physician orders against the schedule to ensure frequency/duration accuracy before submitting to clinical care coordinator staff for evaluation for clinical accuracy.
- Ensures that the visit schedule for the following week is completed by Friday afternoon, allowing clinicians the opportunity to evaluate their assigned visits and coordinate any appropriate changes with scheduling staff, as needed.
- Runs CNA episodes report twice weekly to verify compliance with SVA visit requirements.
- Runs schedule deviation report daily to identify scheduling issues.
Intake Responsibilities:
- Assists the supervisor with the evaluation of incoming referrals for appropriateness for admission to home health services with the agency. Obtains verification of benefits, information on the agency’s status with the payor source (in/out of network), patient demographics and all required clinical support documentation, including the presence of a physician order for care, required to determine if admission is appropriate.
- Logs each referral received in the referral log book and notifies corresponding marketer of referral.
- Accurately fills out an Intake Information Sheet on every pending SOC, ensuring that all the required elements are present. If F2F encounter requirement has been met, intake will obtain a copy of the documentation of this encounter and provide it to the OASIS team for evaluation. Referring entity should be questioned as to why home health services have been ordered for the patient and response should be documented. This documentation should not just be a list of DX. Patient or patient representative will be contacted regarding initiating home health services. Primary language will be determined and documented. Any activated DPOA will either 1) be at the start of care (SOC) visit to receive the required written information or 2) if unable to attend SOC visit, will receive all the required written information prior to scheduling the SOC.
- Accurately and completely enters the patient information into the EMR for all SOCs per agency process. Schedules the SOC OASIS and other tasks to the SOC RN, as mandated by agency processes. Scans and attaches all clinical support documentation to the SOC OASIS task. Verifies eligibility check.
- Coordinate SOC with patient/patient representative and SOC RN. If it is a therapy only patient, will coordinate SOC to ensure SOC date coincides with initial therapy evaluation date.
- If patient is not admitted as expected, EMR will be updated to reflect the non admit status.
Qualifications:
- Home Health experience preferred
- Wellsky experience helpful
- Excellent interpersonal and organizational skills
