ROLE PURPOSE
- Routinely communicates with and seeks guidance from Clinical Manager regarding patient/physician/clinical care team needs and identified issues. Provides report to the Clinical Manager as requested.
- Assists the Clinical Manager to ensure that all referrals within their service area are processed per agency policy. Provides back up services to intake personnel.
- Assists intake staff and the OASIS team with procurement of compliant F2F documentation for SOCs.
- Assists Manager with ensuring that all scheduled patient visits are covered by appropriate personnel. Assists schedulers with management of scheduling issues/conflicts as they arise.
- Tracks and communicates patient lab/test results to the physician and clinical care team. Ensures these priority documents are scanned into the EMR promptly.
- Assists the Manager with management/review of the visit notes within the EMR.
QUALIFICATIONS
- LVN
- Kinnser experience preferred

Job Summary:
The RN administers skilled nursing care for clients in their place of residence, coordinates care with the interdisciplinary team, patient/family and referring agency and assumes the responsibility for coordination of care.
Responsibilities:
- Assess home patients to identify the physical, psycho-social and environmental needs as evidenced by documentation, clinical record, case conference and evaluations.
- Implement/develop/document the plan of care to ensure quality and continuity of care.
- Provide care utilizing infection control measures that protect both the patient and the staff.
- Supervise and provide clinical directions to the home health aide and the LVN to ensure quality and continuity of services provided.
- Provide effective communication to patient/family, team members and other health care professionals.
Qualifications:
- Must hold current Texas State Registered Nurse license
- Must have current CPR Certification
- Must be detail-oriented and have effective communication skills
- Must have at least 1 year of Home Health experience

The LVN administers skilled nursing care under the supervision of a Registered Nurse, for clients in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency.
Responsibilities:
- Under the direction of the RN, assist in identifying the patient’s physical, psycho-social and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.
- Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
- Provide care utilizing infection control measures that protect both staff and patient.
- Assure the continuity of care through delivery of quality patient care.
Qualifications:
- Must hold current Texas Licensed Vocation Nurse
- Must have current CPR Certification
- Must be detail-oriented and have effective communication skills
- Home Health Care Experience
