JOB SUMMARY

A Physical Therapy Assistant who provides coordinated physical therapy to patients of all age groups under the supervision of a Licensed Physical Therapist.

Responsibilities:

  • Administers the physical therapy treatment plan as developed by the Physical Therapist.
  • Accepts clinical assignments that are consistent with education and competence to care for patients.
  • Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Administers non-complex active and passive manual therapeutic exercise, therapeutic massage, traction, heat, light, cold, water and electrical modalities to patients.

Qualifications:

  • Current Texas Physical Therapy Assistant License
  • Current Driver’s License
  • Current CPR Certification
  • Home Health experience preferred

RN – PRN

Job Summary:

The RN administers skilled nursing care for clients in their place of residence, coordinates care with the interdisciplinary team, patient/family and referring agency and assumes the responsibility for coordination of care.

Responsibilities:

  • Assess home patients to identify the physical, psycho-social and environmental needs as evidenced by documentation, clinical record, case conference and evaluations.
  • Implement/develop/document the plan of care to ensure quality and continuity of care.
  • Provide care utilizing infection control measures that protect both the patient and the staff.
  • Supervise and provide clinical directions to the home health aide and the LVN to ensure quality and continuity of services provided.
  • Provide effective communication to patient/family, team members and other health care professionals.

Qualifications:

  • Must hold current Texas State Registered Nurse license
  • Must have current CPR Certification
  • Must be detail-oriented and have effective communication skills
  • Must have at least 1 year of Home Health experience

Home Health Aide – PRN

Job Summary:

The primary function is to provide personal care and related services to the client in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations or problems withe the supervisor.

Responsibilities:

  • Document in an electronic medical record system.
  • Demonstrate commitment, professional growth and competency by completing 12 hours of in-services per year.
  • Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care.
  • Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
  • Promote positive, supportive, respectful communication to patient/family and other employees.
  • Provide an environment which promotes respect for patient, privacy and  property. Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.

Qualifications:

  • Must have current Driver’s License
  • Must have current CPR Certification
  • Minimum one experience in a healthcare setting
  • Must be registered and in good standing on the Nurse Aide Registry
