JOB SUMMARY
A registered professional who provides coordinated care to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment programs based on the medical referral and their evaluation.
Responsibilities:
- Performs skilled and maintenance therapy treatments on patients according to state/federal regulation, their practice act and competency skill set.
- Creates plans of care that are consistent with patient’s illness or injury and particular medical needs, clearly support medical necessity and establishes goals that are tied to function; along with being realistic and meaningful to the patient.
- Submits thorough and accurate documentation of therapy services performed in a manner that clearly demonstrates medical necessity and compliance with state/federal documentation requirements.
- Identifies changes in patient condition and communicates these changes quickly and effectively to the clinical manager.
Qualifications:
- Current Texas Physical Therapy License
- Current Driver’s License
- Current CPR Certification
- Home Health experience preferred

A Physical Therapy Assistant who provides coordinated physical therapy to patients of all age groups under the supervision of a Licensed Physical Therapist.
Responsibilities:
- Administers the physical therapy treatment plan as developed by the Physical Therapist.
- Accepts clinical assignments that are consistent with education and competence to care for patients.
- Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
- Administers non-complex active and passive manual therapeutic exercise, therapeutic massage, traction, heat, light, cold, water and electrical modalities to patients.
Qualifications:
- Current Texas Physical Therapy Assistant License
- Current Driver’s License
- Current CPR Certification
- Home Health experience preferred

Job Summary:
The primary function is to provide personal care and related services to the client in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations or problems withe the supervisor.
Responsibilities:
- Document in an electronic medical record system.
- Demonstrate commitment, professional growth and competency by completing 12 hours of in-services per year.
- Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care.
- Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
- Promote positive, supportive, respectful communication to patient/family and other employees.
- Provide an environment which promotes respect for patient, privacy and property. Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
Qualifications:
- Must have current Driver’s License
- Must have current CPR Certification
- Minimum one experience in a healthcare setting
- Must be registered and in good standing on the Nurse Aide Registry
